AMERITAS

In most states, when using an in-network provider, claims are paid at the Maximum Allowable Charge (MAC) claim allowance which is the maximum amount a network provider may charge. If using an out-of-network dentist, the claim allowance is considered at the Maximum Allowable Benefit (MAB), which is equal to the lowest network contracted fee in the ZIP Code area.
With no Coordination of Benefits (COB) in our plans, we essentially ignore any other Explanation of Benefits (EOBs) or documentation submitted, therefore we process the claim as primary. b. Because we always pay primary, you can have an Ameritas plan with any number of other dental plans.

For a plan that is paying benefits to an in-network provider, there should be no “balance billing”, rather the provider would bill the member the portion of the claim that the plan does not cover. (i.e. Ameritas covers 80% for a filling, member is responsible for 20%) b.  When paying benefits to an out-of-network provider, the plan pays the insured percentage based on the Maximum Allowable Benefit (MAB) which is the lowest network rate in the ZIP code area. Therefore, if Ameritas covers 80% for a filling, the member is responsible for 20% plus the additional amount charged above the MAB. c. Check out the agent marketing site for state specific details.

Prudent Pet provides pet insurance that reimburses eligible veterinary expenses when your dog or cat experiences an accident or illness.
Yes. You may use any licensed veterinarian, specialist, or emergency clinic in the U.S. or Canada.
Conditions that showed symptoms or were treated before enrollment are considered pre-existing. However, conditions that remain symptom-free for 365 days may be considered cured (excluding knee and ligament issues).
Pricing depends on your pet’s age, breed, location, and selected coverage options. Plans are customizable to fit different budgets.
After the required waiting periods: • 5 days for accidents • 14 days for illnesses • 6 months for knee and ligament conditions
The annual limit is the maximum amount Prudent Pet will reimburse during a policy year.
No. Prudent Pet does not deny coverage based on your pet’s age or breed.
Coverage includes accidents (e.g., broken bones, poison ingestion) and illnesses (e.g., cancer, infections, allergies). Many plans also include hereditary and congenital conditions.
No. Prudent Pet will never cancel coverage due to claim activity.
You pay the vet directly, then submit a claim using your receipt. Once approved, Prudent Pet reimburses you by check or direct deposit based on your plan’s reimbursement percentage.
